Yes, that is a frustration for VoiceOver users. There is a button to "Select all" messages when editing a message list, but after doing that, there is no option for deleting the selected items.
We need to make a suggestion to the Apple accessibility support team.
The other frustration I have with mail on iOS devices (not limited to visually impaired people) is that all mail folders and sub-folders appear as expanded when expanding the root folder. What a mess that makes and a pile of sub-folders to navigate past when looking for a particular folder. Not very elegant.
I think the problem of deleting all mail at once is an issue on iOS devices. I don't think there is any easy way of doing this.

Actually, now that I'm thinking of it, I believe that there might be a "Move" option and one can then move items to the Trash. I'll have to try that. Not as easy as hitting Delete since it takes more steps, but maybe it can work.

If you're looking to delete multiple messages contiguous or otherwise, there's always the multiple selection trick with Voiceover. Just select a row and use voiceover commands including VO+command+enter to fine-tune your selection. If you're however looking to simply delete all messages in a list, the aforementioned command+A will do the trick nicely.
A little tip, try VO+F6 when working on selecting items anywhere.

Hi, do you mean deleting all mails from the native mail app on MacOS? Just CMD+A then CMD+backspace. Sorry if this is not what you meant.
