I'm going to ask here because I've been emailing Apple Accessibility all morning.
Sometimes in documents, VO stops speaking. I need to turn it off sometimes in order for it to speak again.
Anyone notice this?

Yes. This has been a very irritating problem for the past several OS X releases for me. It occurs with all of the TTS voices that I have tested. Like you, the only solution seems to be toggling VoiceOver off and back on again.
If anybody knows of either a fix or a voice which doesn’t have this problem, I would love to hear!

Another thing I've noticed is VO wouldn't type when quick nab is on.

Yeah unfortunately I've been dealing with this one too. It will sometimes let me read whole lines but I can't move character by character until I restart VO. I have no clue what triggers this.
I'm going to go email them as well!

Why don't you just contact apple accessibility on the phone instead of emailing them? They can help you directly.
I ran into this in TextEdit
I don't know what apps you are using, but I once ran into this in TextEdit. It seems to happen when the document is more than a single page, at which point VoiceOver stops speaking contents after page 1. Turning VO off and back on temporarily solves the issue, and I think switching to another app and switching back will also work around the bug, though I don't remember for sure.
In TextEdit, I can switch from "wrap to page" to "wrap to window," which causes the document to display continuously without any page dividers, and the problem does not occur. Depending on what I am using TextEdit for, this is often sufficient, but if I am working on something I need to print out, it is often necessary to use the "wrap to page" mode in order to ensure that the text on the page will be properly lined up when printing, that page breaks are at the right locations, and so on.
