Description
There are some inconsistencies of behavior when switching voices in VoiceOver settings > Speech which can result in VoiceOver not automatically switching to your new selection. You may need to use the language rotor to force the switch or select a different voice and then reselect your first choice.

Can't change voice or speaking rate at all
Once I installed iOS 9.0 the VoiceOver voice was distorted and the speaking rate very fast - nearly illegible. I have been unable to change the voice or rate in Settings. It allows me to select a voice, but the "selected" voice is NOT used. I have found no way to get the "selected" voice to be used. Restarting the iPhone (6 Plus) has no affect.
I have not installed 9.1 yet ... will it make it worse or better!?
