New bug in iOS 26.4 that was unheard of in the past

Hello everyone! Continuing to be distraught about Apple’s accessibility team not doing their job as properly as they used to do back in, let’s say, IOS 10 times :(
I just updated my phone to the brand new IOS 26.4 just to find that, at least in the notification center, VoiceOver doesn’t pronounce the source of a notification, aka the app which it came from. How we even got to those bugs on IOS in 2026?

I am unable to duplicate this on my 16+ running iOS 26.4. Have you restarted your device after the update?

I didn’t confirm that.

I’ve had an iPhone 16 Pro and iOS for two days. All notifications are read correctly.

I have been using 26.4 update since yesterday morning, I haven't encounter something like that on my 16 plus.

Notifications are read normally on my iPhone 16.

I'm sorry, I can't replicate this error.

I've been reporting bugs to Apple for the whole iOS7 and 8 branch, till you could do it from bugreporter.apple.com - and until I could use my phone for beta-testing. Now I no longer can.

And what I noticed, is that there are too many machine-related bugs when talking about accessibility regressions. Every machine works on its own, and I find difficult to confirm - unless bugs are really evident. This one of notification is not on mine, is not on other commenters machines, and who knows? If it could be possible to isolate them better!

Suggest you ask Siri to reset the phone. Siri will ask to confirm then reset. May sort this as I can't reproduce it either and it seems nobody else can.

Yeah, thanks guys, that was a passing issue, as soon as I restarted Voiceover, the bug was gone for good. I wish it’d be related to most bugs

The first is automatic, then I immediately do one myself. Fix some weird things with voiceover, even on macos :) yeah, macos too!! It's not that broken. Just a lot. And I use it.

Even me here in South Africa, on a 17 ProMax, did not experience such a bug. I've been using IOS 26.4 since it's beta days

I've got an iPhone 16, and my phone always tells me what app notifications are from. Maybe it's a setting of some sort that is impacting it?
