'I'm currently using telegram on Iphone 17.
Recently, when I tried sending a text message, I noticed the voiceover accessibility is failing horrifically.
I couldn't find the text edit box to send the message.
I got one, by clicking on reply to an existing message.
But once I typed my message, I couldn't find the send button.
I had to essentially turn off voice over, randomly tap on things, turn voice over back on, turn on screen recognition, and somehow found the send button.
Is anyone else running into this issue?
If so, are there any work arounds?

Experiencing sane
I have very recently begun experiencing the same issue with Telegram that you outlined above with no means of working around aside from what you explained in order to bring up the text box and find the Send button using screen recognition. This doesn't appear to have begun with a particular version upgrade Of Telegram or iOS, and thus is quite frustrating.

As far as I know, the telegram application for iOS is not very accessible. However, someone has made a really amazing script for the web version of telegram using Safari on iOS. Check out the link below.

Workaround
The compose message edit field is at the bottom center of the screen, at a similar location to the Messages app or Whatsapp. Turning off VO and tapping that location should show the keyboard, and then you can type normally once you turn on VO. As for the send button, if you use BSI, the three-finger swipe up gesture does still work to send a message.
