I'm not sure when this started happening, but now when I open Discord on my iPad Mini, I get the list of servers and channels, and the VoiceOver scrub gesture or Space+b does not close this screen and take me to the message list like it used to. Also, selecting a server in the list often opens the user profile or does something else random rather than changing the list of channels to the ones for that server. When I activate a channel, the message list opens, but when I press Space+l or Space+dots 4-5-6 to go to the top or bottom of the screen, I'm back in the server and channel list. These issues have made the Discord app pretty much unusable on the iPad with VoiceOver, and turning on the old server list option in advanced settings doesn't do anything to fix this. It seems a lot less broken on iPhone, although opening a channel near the bottom of the list often opens something random. I'm running the latest version of iPadOS and I have the latest Discord update.

On iPhone is indeed quite well and I use it everyday, have you tried explore by touch on the iPad? Cause like the scroll bar is reversed so 0% is the last message. VO tries to put it what it thinks it full so 100% so it constently auto scroll backwards which creates all sort of weird usses that's why I use explore by touch most of the time as the actions are broken.
Discord have an accessibility issue form you should fill out on their website too, everything mentioned above was reported a few days ago to them by this form.
Landscape Mode
Putting the iPad in landscape mode makes it better, since if you scroll down before selecting a server further down the list it works. And the message list is also easier to find by going to the bottom of the screen although the server and channel list still doesn't dismiss.
Explore by Touch
Explore by touch doesn't fix the server selection issue which was one of the most serious ones.
