Have you looked at atomic wallet? They do have a mac desktop client, though I cannot speak about it from personal experience. I do know that the iOS version is almost fully accessible. The only thing I can't seem to do is filter the listed crypto currencies on the wallet main screen. Everything else though appears to work nicely.

Sorry, I wish I was more help. It's been a couple of years since I tinkered around with crypto. I even wrote a fun pulp/noir blog about it.
I’ve cashed out of crypto, and here’s my reason: Accessibility isn’t a high priority for these people. I don’t want to lose access to an investment because some green programmer fresh out of high school released an update that totally removed what scant accessibility had been available in the prior release. This has heppened to me with the Trust Wallet iOS app and both the CoinBase and Gemini websites, and I don’t get a lot of sympathy when I reach out to their support. The only workaround is to forego all updates while I hold an investment, and that’s not acceptable to me.
So, anyone in crypto, I wish you good luck.

Have you tried Cake Wallet, available in the Mac AppStore,, the iOS version is pretty accessible with VoiceOver.
