I’m using iPhone from more than two years, I reported this issue with the Apple Accessibility supporting regarding poor language support for Indian regional languages I informed them regarding this more than five times including providing the video recording for them. But still, there is no improvement in the auto along with switching VoiceOver for Indian regional languages. Only Hindi is fully supported, but not other languages.
I’m really not sure what’s the issue with the Apple on this. Because in android this feature is available for a long time and it is working perfectly fine.
In android there is an application called auto TTS, if you select two languages, for example, English US and a Telugu, it will automatically switch the language where the Telugu text is found on the screen.
If anyone develops an application based on the same principal, that will be really helpful. I completely lost. Hope on the Apple on the same.

Have you tried adding the languages in Voiceover Rotor and browsing the content of the languages concerned? I have tried adding Tamil and Marathi on the rotor and navigated BBC website of these languages. For Tamil, it just read the content for me without the need to switch the rotor, whereas for Marathi I had to switch it but thereafter it read the content to me. Of course, there may be a whole lot of other factors that may be going against accessing the language contents smoothly at your end, including the way in which the content is formatted.
Also, if you are looking for a third party solution, Espeak Ng should be able to do the job. True that it does not sound natural, but something could be better than nothing.

If you observe the Hindi language, switching, and if you compare that with Marathi. In Hindi language switching VoiceOver automatically switches the language once it detect the Hindi letter. But in other regional languages, it is not happening. We need to intentionally change the voice and read that language.
The main problem for me was, when I reading the messages if there is any Telugu text in between that, it is not reading at all. For me, I’m not sure in which language the text was written without knowing that it’s very difficult for us to change the long wages, right?
I am using the speak also, but I am facing the same issue with the ESP. The one more problem I observed with the ESP was it is not reading the description of the images.
