OK, this is a horrible ordeal to do manually. Someone else can tell you about that app that can do it for you.
Open your picture. There is an edit option at the top. From there, there is a markup option. At the bottom there will be an add option. After that there will be a description option. That is what will give you a text box that will ultimately add something to the meta data of the photo that VO will announce when you are on the thumbnail or the picture.
I don't find those options for a video, so I don't know how you do them.
* That app that will, as far as I have read, put an AI generated description of the photos is, PiccyBot, and there's a thread on it.
You can also change the file name by sharing it to the Files app, then change the file name of the photo in the Files app, then share it back to the photos app. I don't remember if VO announces the file name at the thumbnail level.

Yes, that is a bit of a tedious process, but at least it can be done so that we can easily scroll through our photos and tell what they are.

Unfortunately this doesn't seem to work for videos.

Following your steps, there is no "Markup" option for videos. I would love to be able to properly label my videos so that I know which is which. Is there a way of doing that? One would think adding descriptions for photos and videos would follow the same process.

I know of no way to put a description on a video in iOS. You could try going through the process of sharing it to the files app, renaming it, then sharing back to Photos. I'm not sure if the Photos app reads the file name though. A while back, I started keeping all my videos and photos on my main computer where I go only by the file name, and I've never sent one back to my phone.

Why can't you use the VO labelling gesture for this? I mean, it will still be very tedious work, but doable.

To do this, open your photos app and navigate to whatever you wish to rename. With VO focused on a picture or video, double tap (and hold) with 2 fingers. Do not let go. You will hear 3 short soundbytes followed by a 4th alternate soundbyte.

Now you will be in the rename dialog. Do this for every single photo, video, etc you want.

HTH.
