Hello,
I've just recently experienced where VO has stopped speaking but is still running. This is when I've been in the music app, so has anyone else experienced this? When it happens, none of the gestures work either, and no sounds or haptics, too. I've had to turn off VO and turn it back on again, but in the most recent issue of this problem, it didn't help. I had to close the music app, and while a song was running, push the sleep/wake button, pick up my phone to get VO to speak, and go back into the music app just from the lock screen. Then, VO works fine ... as it used to. Keep in mind that my iPhone's one of the older models, 8+ with home button. Could this be the cause? I've not used all of the 64 gb up yet/try not to use it for more than I need, that is.

Don't worry, it's probably…
Don't worry, it's probably not just because your iPhone is an older model. Try a hard reset: quickly press and release Volume Up, then Volume Down, and hold the Side button until the Apple logo shows up. Also, check if your iOS is up to date; updates often fix these glitches.
Keep an eye on your storage, too. Even if it's not full, low space can cause issues.
Same issues
I'm experiencing the same issues.
VO suddenly stop speech output but waiting 15-30 seconds usually brings back speech.
Toggling VO off/on again always works for me.
I haven't found any pattern to when this happens though.
I'd say it's a bug because I've seen multiple people report the exact same behavior.
It happens to me once in a while
VoiceOver just stops speaking for reasons unknown.
It's not so frequent, so I don't get very annoyed.
This happens to me on Discord.
I'm new to that platform, so maybe that's normal, but it's still annoying.
Seems to be a loading bug
As far as I can tell it seems like Voiceover has to load or wait for a page or app to load before it will start working. I guess the speed for which Apple is known is for sighted users.
