I hear this is different for newer phones though. Just yesterday I activated Emergency SOS by mistake on my iPhone 7. I won't get into an explanation of how this happened, as I need to start my laundry in a bit. But it turned out well in the end, and at least I have confirmation that this feature indeed works with VoiceOver on my current phone.
It's very easy to trigger EMS, I've done it a couple of times, once half asleep, the other it was buried in my purse, thanks designers for thinking women don't need pockets. It buzzed, thought nothing of it, then I heard this siren ring, as I realized, oh hell, and grabbed for it. Thankfully the dispatchers understood it was a mistake and no cops were summoned. Hell even my old man called them. Note, they will call back immediately. The action button on the 15 series I don't know if that will do it. Also, the silly/stupidest question is the one you don't ask.
