I have a Word Document, and Voiceover will not Navigate by Headings

Hello friends. I have a Word document that I moved onto my iPhone. It was set up with headings when I created it specifically for navigation purposes. Unfortunately, now I cannot get any of my writing apps to navigate by headings. I have tried Word, Voice Dream, and Notes. I had removed Pages off the phone because I never used it, but can easily replace it if necessary. I'm running an iPhone 13 with iOS 16.6.5.

If I'm doing something wrong, or if I should try creating the document a different way, I would be delighted if anyone can suggest another way to create a document with headings.

Was this document originally made on a PC? A Mac? I do know that transferring text documents from one OS to another tends to screw with the original formatting. If you have access to a Mac, I would recommend converting the document to Pages on Mac, then transferring to your iPhone via iCloud, and then opening it up in Pages for iOS.
If you have a PC, and access to Microsoft Word, then just make sure you have the document setup the way you want in Word, and then again transfer to your iPhone via iCloud. And finally try using Microsoft Word for iOS once again on the iPhone.

I set it up in Word correctly on my PC with recognized headings. I then put it in my OneDrive folder, opened OneDrive on my phone, saved it into my files, and then opened it in Word. Let's say that the headings are not happy. Smile.
