VO+Tab, the VoiceOver passthrough command, or "ignore next keypress" command, appears to be non-functional in Mac OS 12.2.1.
To reproduce, open any app or window. Press VO+Tab. Then try to navigate with VO+Right or Left arrow. The expected behavior is that the navigation command should be ignored. However, on current Mac OS, the navigation command is not ignored.
I first encountered this with invert colors, and mentioned it in this forum topic. However, I've now discovered this is not specific to invert colors and seems to be a problem with the passthrough command in general.
I'm on a 2019 Mac Mini. Resetting the NVRAM has no effect. Please let me know if anyone else can confirm this issue. Thanks.

I actually think this has been broken for a while
Yes, I can confirm this issue. My work computer is a 2018 Macbook Air running Monterey 12.1 and the pass-through command is definitely not working. This is especially annoying since my company does a lot of work in G-suite and that command is practically required to navigate efficiently through the Docs and Sheets interface. I've been putting off updating my personal computer due to all the issues reported on Monterey ... guess I'll keep waiting then.
I already reported this to…
I already reported this to apple last year when I first noticed it. Here's hoping they fix it. Over all though mac os 12 works grate, except for that little quirk.
Good to know
I upgraded from 12.0, so I wasn't sure when the issue was introduced. I did not see it mentioned in the AppleVis bug tracker database. Maybe the editors can add it.
Anyone tried redefining the hotkey?
Shrug. Who knows? Might work.
Still broken in 12.6
This is still an issue in the current Mac OS 12.6.
This issue renders Google Docs pretty much unusable. The only workaround is to set the VO key to Caps Lock, but muscle memory pretty much makes the Caps Lock solution a non-solution, at least for old timers like me.
Could the AppleVis editors please open this in the bug database? Thanks
Fixed in Ventura
Ventura resolves this long-standing issue. Hooray!
That is so weird!
Wow, I never was able to reproduce that. Not even in Monterey, and believe me, I would have noticed it, as I do a lot of ignoring. I use both ProTools as well as Logic, being I'm a sound engineer, and a lot of commands rely on things containing CTRL+Option. I actually finally wound up changing the VO modifier to caps lock, as it was becoming so annoying having to do that before so many commands. I'm curious if this bug only cropped up with specifically CTRL+Option+Tab, but not when using caps lock, as again, I tell ya, even in the exact builds mentioned, I've never ever been able to have that break on me. I guess that's a good thing, but yeah. Maybe I'm just lucky, who knows.
It broke iwht capslock and…
It broke iwht capslock and control option for me. I need to test, I think for me it is still broken, that is with the latest mac os 13.
