With grouped navigation enabled, when encountering an item that requires interaction, swipe right with two fingers to interact, at which point swiping will only show content within that item until you stop interacting by swiping left with two fingers.
Possibly it's too many years of muscle memory, but this new navigation style just doesn't do it for me.
It just feels like extra work for thong in return, although I guess that my main issue is that this style makes it impossible to use your iPhone one-handed.
The Group Nav feature's more for those who are use to how VO is on the Mac side of things.
I myself don't use Group nav on my iPhone, though I do on my iPad mini.
BTW, if you're using an external keyboard, VO-Shift +Down/Up-Arrow starts/stops interacting with items.
You can add a toggle switch for Flat and Group Nav modes in either the main rotor or VO Quick Settings, if desired.
