Ok decided to make a folder from two apps. iOS named it productivity which wasn't what I wanted. So, by using a two finger double tap and hold I edited the folder and renamed it to ocr. Now here is where it has got weird. Every folder I now have on my phone now says ocr, and then the name of the folder when I enter the folder. So for example I have a weather folder before i would select that folder and it would say weather. Now it says ocr, weather. I removed the two appps from the new folder and it deleted it. However, it still says ocr at the start of every folder I now have even though that folder has gone. Has anyone else seen this? Or has anyone got a clue how I can stop this. I tried renaming one of my folders back but didn't alter things. Very odd. I also did a hard reboot. iOS 11.4.1 iPhone 8. Any thoughts/help appreciated.

Naming folders

That is actually not the correct way to name a folder.
By naming the folder Ocr using that method, VoiceOv erassumes each folder should be called OCR folder.
Here's how to fix.
1. Double tap and hold on each folder
2. Remove the label
3. Hit save when finished.
To delete all, set rotor to edit, swipe down till you hear select all, and double tap. Then from your keyboard hit delete.

So if renaming folders above is the wrong way then does anyone know the correct method for renaming an ios folder to avoid what happened here?
Use the edit mode
On the home screen, swipe down until you hear VoiceOver says edit mode, then double tap. Find your folder and double tap on it, just like you would delete an app. Now, on the left there should be a folder name field. Double tap on it and simply replace the name with the name you want. Hope this helps.

Not Working
I am about to go crazy because of this same problem. I have tried the fix given above, but I must be doing something wrong. No matter what I name a folder, several folders get renamed and they all say something before the title that I gave the folder. So when I look around on my phone, several folders are named the same because of this problem. I tried editing the folder names, but one of the steps says to select all, but I am not figuring out how to do that after trying to relabel each folder. Can someone please help explain how to fix this step by step? I am using IOS 26.5 on a iPhone SE 2nd gen.

Try these steps:
• navigate to a folder on any of your home screens.
• with VO focused on the folder, triple tap, and then choose the, "Edit Home Screen", option.
• now that you are in Edit mode, double tap on that folder. If VO focus goes elsewhere, just navigate back to that folder, and double tap on it.
• Inside the folder, while and edit mode, swipe all the way to the left which should take you to an edit field with the current name of the folder. Double tap inside the edit field, then do your rotor gesture until you find the word, 'Edit'. Now swipe up or down until you find, 'Select All', and double tap on it.
• Double tap on, 'Delete', on your keyboard to erase the previous label, and then name it whatever you want. Double tap on done (it will be where enter usually is on the keyboard), and then do your home gesture to get out of edit mode.

Okay, I Finally Got It!
My problem was myself, I was not getting the triple tap to work correctly, LOL. It took some practicing to get it right. I did a few folders renamed to what I want and it did not rename all of them like it had been doing. I still have a little problem though. I made a folder once and I named it with an Emoji in it, and now when browsing around on my phone, Voice Over constantly announces that folder name with the Emoji when I touch on any of the folders. Even the page selector says that folder name, and the funny thing is that the folder is no longer on my phone, yet it still says it. I have spent so much time trying to figure out how to make this stop with no luck. Any ideas on that issue?

I have never experienced any of this, so I'm not really sure what to tell you, other than to erase all of your folders and start fresh. 🤷

Can't believe it was nearly 8 years ago I uploaded this post and it appears it is still an issue. Doesn't look good for fixing the notification issues lol.
Sadly, I’ve tried that already.
I went through and removed all my folders, starting over, and it did not work, the stuff just keeps repeating, no matter what I do. I really don’t wanna wipe my phone clean, but it might come to it since there’s apparently not a fix for this. There are no folders now, but I still keep hearing the same thing when I touch on any app or using the page selector.
Reset Home Screen layout
Have you tried resetting your Home Screen layout in Settings > General > Transfer or reset > Reset? That's one thing I'd suggest before factory resetting the iPhone.

...and if that does not work, you can always try resetting all of your settings. It's not quite as dire as doing a factory reset, but it's definitely time-consuming all the same. It basically resets everything within settings, including application settings, back to their Standard defaults.
Hopefully you will not have to do this, but if you do, it may be what fixes the folder naming issue.

With a lot of work, we have come up with what I feel is a thorough description of the problem that I’m having so I can send it to Apple accessibility and maybe get some kind of answers, not sure if this is exactly what’s happening to other people. Here is the letter we have composed: Hello,
I am a totally blind iPhone user who relies on VoiceOver for all interaction with my device.
Recently, I created a new folder by combining two apps. iOS automatically named the folder “Games.” I then edited the folder name and changed it to “♥️ Card Games ♥️” because I already had a folder named ‘Games’ and I wanted a more descriptive name for this folder.
After making this change, I began experiencing what appears to be a VoiceOver labeling issue. VoiceOver now frequently announces “♥️ Card Games ♥️” before the actual name of many unrelated interface elements throughout iOS.
For example, when navigating folders on any screen, VoiceOver may announce “♥️ Card Games ♥️” followed by the folder’s real name. I have also observed this behavior in other locations, including within apps and even in the App Switcher. The issue is inconsistent. Sometimes the extra phrase is announced, and sometimes it is not.
The behavior does not appear to be tied to a specific VoiceOver voice. I have tested multiple voices, including Samantha, Alex and Evan, and the problem persists across all of them. There is no noticeable pause between “♥️ Card Games ♥️” and the actual label being spoken, which makes it sound as though the text has become attached to the accessibility label itself rather than being spoken as a separate notification or hint.
To help illustrate the issue, when I copy text from certain locations, such as Notes or Draft Pad or even in the Mail app, the copied content may begin with “♥️ Card Games ♥️” even when that text is unrelated to the folder. This suggests that the folder name may somehow be leaking into accessibility labels or VoiceOver announcements beyond the folder where it was originally created.
I was experiencing this issue on a iPhone 13 running iOS 26.5.1, but the behavior also is happening on my brand new iPhone 17E and any other devices running earlier versions of iOS that are signed into the same Apple account. I have been unable to find any setting or action that resolves the issue.
I even tried resetting voiceover settings, and I even reset the home screen layout with no results, the problem still exists. I would appreciate any assistance in determining whether this is a known VoiceOver bug, an accessibility cache issue, or another problem affecting accessibility labels within iOS.
