Taken from Found an Accessibility Bug in iOS, iPadOS, macOS, watchOS or tvOS? Here’s How to Let Apple Know and Why You Should:

  1. A brief description of the problem.
  2. State the frequency of the problem - for example, does it always happen, only sometimes, or very infrequently.
  3. Give details of the steps necessary to reproduce the problem.
  4. State what you would expect to happen after taking the steps above.
  5. Explain what actually happens after taking the steps above.
  6. Give details of the software and hardware that you are using.

This should be all that the Accessibility team need.

Hi there!

I agree with the comment above, but, I also think you should get a screen recording, and or a screenshot, or both. Tech support loves screenshots and screen recorddings. Usually, when I tell them I have one or both of these, and ask if they want them, they immediately say yes. Although 99% of the time, I just send them with out being asked.
These things give a clearer picture of what's going on, and sometimes there are things that VO. can't read or see, but a sighted person can see. This happened on a few screen sharing sessions with tech support.

Also, be nice about it. Give as much info as possible. I get being frustrated, believe me. But, I try and always treat people the way I'd want to be treated. The old saying that you attract more flies with honey than vinigar has a lot of truth in it. Not saying you aren't nice, but just try and keep that in mind.
