The same thing happened to me. This is what I had to do to correct the situation. First, I added the punctuation function to my rotor. Secondly, I turn off voiceover, turn voiceover back on, and then was allowed to change the punctuation Setting within the rotor option. I hope this is helpful.

I'd previously set up a number of activities, each with a different voice. When I updated to iOS 14.3, the voices disappeared. In their place were two options in the activities rota - default and programming. It's this latter which has all the punctuation in it.
If you go into settings, accessibility, voiceover, and finally activities,you can one finger swipe down on programming and delete it.

Thanks, but that didn't work for me. Nothing changes when I double tap the choice from the rotor. Better too much speech than too little I guess.
