I've read online that VO is supposed to announce what the iPhone camera is focused on. However, I cannot get this to work. Can I be doing something wrong?

Is your screen curtain on? If so, make sure to turn it off, because the screen curtain disables the camera along with the screen. Also, the camera really only recognizes faces, so if a face is not in the photo, you will hear no feedback. You also want to make sure you have adequate light for the camera to function.
You need to also make sure that mode is set to photo, not video, and make sure that the correct camera is active. Also, I believe if you try and take a picture from the unlock screen, VO will not provide feedback.
Hi. No I'm afraid that the camera won't do this. It will sometimes say the word "Autofocused" to let you know that the image is in focus, but it won't announce objects. If there are faces in the shot though, it will tell you that, which can be useful.
If image recognition is what you are after, then I'd recommend apps such as TapTapSee and CamFind. These though are for identifying objects for you, not taking photos of objects to keep.
