I'm thinking about getting Apple Watch 6. Could you share your opinions about 40mm vs 44mm?
I'd imagine screen size doesn't matter much for VO users like iPhone screen size?
In that case, should I go for 40mm and save some bucks?

The traditional philosophy on this is that men should go with the larger size and ladies should get the smaller size. Now, I know there are some exceptions to this as I know women with the larger watches and men who prefer the smaller size, but I think it is typically true. Also, as you are dealing with a small screen to begin with, having a larger screen may help you if you have larger fingers. I have a friend who has a smaller model. Trying to perform certain gestures on her watch is quite difficult for me. I have a larger model and find the larger screen makes it easier for my big fumbling fingers to perform these gestures.

Watch sizes
I'm one of those ladies who prefers the larger ones. I don't mind it looking a little bigger if it means getting more battery and a bigger surface to perform gestures. These things are tiny enough as is.
James, in any case, I would seriously consider the Watch SE or the series 4 or 5. The Series 6 doesn't bring much to the table and isn't worth it unless you really, really, really want pulse oxymetry or blue aluminium.

I agree with Pepper Fox. I am also a lady and much prefer the larger screen. Although I have small hands and wrist size, I don't feel that the larger watch size is in any way looking too large for my bone structure. I also get the privilege of being able to have the additional screen space and more than one-finger gestures are simply easier.

Not really. 40 or 50 when watches were made the answer was yes. Now because they were digital no. I have the 40 and works well. If you have big arms maybe the 44 maybe is for you. In general is a choice on the person and not gender anymore.

44mm all the way for me
As already mentioned, you get better battery life with the larger watch.
How much that will matter will depend upon your use case - if you want workout tracking, sleep tracking, navigation, and audio playback, then it's the 44 all the way.
The introduction of the VoiceOver Rotor with watchOS 7 has also made me very glad that I don't have the 40. I'm already liking having the rotor, and know that with my fingers, turning that rotor on a 40mm watch would not be fun.
