Hello, what are the benefits of getting an Apple TV fourth generation? What should I consider before buying it? Is it really accessible?
In advance, I thank you for all your comments. I really appreciate it :)

Hi there! On the whole, I think the new Apple TV is vastly superior to the 3rd generation model. The touch remote is a breeze to use; had it mastered within half an hour. I love that Apple TV now has apps that we can download. It makes it more flexible, and also, you don't have to worry about hiding all those apps you'll never use. Eventually, this may mean that developers will make VO friendly games for us to play on the Apple TV. All the apps I have tried work very well with VoiceOver; some aren't quite perfect, but everything has been extremely useable. The only minor complaint I have with the new Apple TV is something that probably wouldn't even be a factor for most people: I'm disappointed that they took away the option to use album art as our screen saver. I just use the built in arial screen saver, but the album art option seemed a lot more fun to me, especially when music is playing.

Is there also an option to set it up with VoiceOver? What Applications do you have?

Yes, it's absolutely possible to set it up with VO. It's even easier if you have an I Device handy. There have been several podcasts showing how to set up the new apple tv. I recorded one myself, but never actually posted it anywhere because it wasn't my best recording effort and I had lots of shiny destractions going on. Anyway, the point is that you can definitely set it up using VO, and getting VO started is way, way easier than it was on the 3rd gen apple TV.
