nvda remote on Mac os

Hello,
Using the NVDA remote app, which was originally made for iOS but also works on m1 Macs, I keep running in to an issue that is really bothering me. Not sure if it is a bug, or something I am doing wrong. Seems that when I am using NVDA remote, somehow the ctrl + alt keys keep getting locked. At the very least control is somehow locked. This makes getting things done almost impossible. It doesn't always happen right away, so I am wondering if there is a Mac keyboard command that is conflicting somewhere. When this happens, even when I am sitting in front of my Windows desktop, I continue to have the issue, and the only way to fix it is to completely restart the computer.
I'm really hoping that someone can tell me what is going on, or what I am doing wrong, as it would be awesome to have NVDA remote working on the Mac, and not even need to have Windows running via utm. Even if I do decide to keep Windows, it would be great to not have to load it up every time I want to remote in.

The only thing I can think of at the top of my head is to make sure VoiceOver is turned off when interacting remotely with your Windows computer. Because the Alt key is by default equivalent to the Option key, I'm wondering if VoiceOver is intercepting these keystrokes, as they involve pressing Control and Option, one of the VoiceOver modifiers.
