Before I report this to Apple, I'm wondering if I'm missing something or anyone is able to confirm this?
Previously, "change pitch" for capital letters did not work with Eloquence, but did work with Nuance voices. Now though, it appears the functionality is broken across the board, unless there's a setting I'm missing somewhere that needs to be tweaked. To replicate:
1. Type a capital letter
2. Move the cursor to the capital letter and see if VoiceOver changes pitch (it should).
I'm using Samantha enhanced on MacOS 13.5, and have capital letters in verbosity under the text tab set to "change pitch".

Not just you and a potential workaround
I've been experiencing this issue as well and filed it under FB12334922 for the Sonoma publuc beta, feel free to reference this as well if you use the beta, but I've noticed this issue on Mac OS 13 as well.
And just now I accidentally might have stumbled across the contributing factor for this.. I was running my ventura installation with the Polish region, which in turn uses the Polish number format. In Poland we use a comma as the decimal separator instead of a dot like the US does. I just had to change it to the US number format, and suddenly my pitch changes started working again.
So, try going to system preferences>general>language and region, and try changing the number format to the US one which uses dot as the decimal separator, hopefully it'll bring the pitch changes back for you as well. If it does help and you reach out to Apple point this out in your message.

Kudos for finding such a delightfully obscure workaround! Changing the number format solves it for me as well, as we use the comma for decimals in Denmark.
