Ghost element at the lowest interaction layer in textedit text field

Hey,
So recently very strange things started to happen inside TextEdit mainly, I haven't tested or tried this elsewhere.
Basically I was editing a text file, plain text not rtf. Out of nowhere, at the end of the document, when I'd interact in the text field, normally it stops there but at that moment it gave me an untitled image out of nowhere creating another layer of interaction inside the text field (announce groups setting). No description for that image, the rest of the document was perfectly normal, and obviously when I restarted VO the issue was completely gone.
It was similar to how the braille caption is shown and visible to voiceover which is a feature not a bug although poorly undocumented in my opinion and they could have created better way for the user to access that.
Not expecting anything to come out from this post, just yet another VO weirdness I don't have the willingness, time or proper documentation to report anyway.
The thing that bothers me is that it's not the first time. I have all the visuals, tiling and etc completely off, and tried with screen curtain off and on.
