Voiceover only working when I engage SIRI

So I've been having issues with Voiceover not coming back up after I get off a call, but I simply shut down VO and bring it back up. A pain, but it works.
This morning, Voiceover is only working when I ask SIRI something, and I can get voice on the SIRI screen.
But as soon as I close the SIRI screen, Voiceover goes away.
I have powered the phone off and back up, I have stopped and started VO with SIRI, cycled VO off and on with triple tap, nothing is working.
I have made no updates recently, nor any changes.
I run my business with this phone, and I am totally stuck today without Voiceover.
Any ideas greatly appreciated.

So, I remembered two things.
1. I had my output set to an external blu tooth speaker from last night in another room.
2. I had tried using Shazam to identify a song by having it listen.

When I did the Shazam thing, I got no feedback, so I just figured Shazam was tripping out on me.
I only later tried to check messages on my phone and realized Voiceover was not working.
At which point I was trying to get the phone and/or voiceover to reset.
Once I remembered the shazam request I had made, , I turned blu tooth off, and voiceover is now working normally.
Is this 100 percent operator error, and I should have known what was going on, or is this some kind of weird bug with the blu tooth device trying to pick up input from Shazam?

Hi,
Not that it necessarily matters, but what model is the phone you're working with?

What's the ios version? I ask because ios 15.1.1 was recently released, but is only available for models in the iphone 12 and/or 13 series.

12 mini, with 15.1.1.
I think others are having the bug with getting off a call and having to reset voiceover, but I've seen no solutions to that one, and it seems that would be pretty wide spread.
The thing I did this morning was just a weird sequence involving blu tooth cross threading, and should be pretty rare, but interesting it only responds with VO while SIRI is up.

Hi,
I’m having the exact same issue, on my current 12 pro max, which is also running ios 15.1.1.
I’m getting the 13 pro max soon, & will report back here, once I'm able to test with the new phone.
Have you informed Apple accessibility of your issue?

Yeah, I called them twice now, months apart.
Same as usual, we have no solutions, engineering/accessibility has been notified, etc.
I know they have a lot of fish to fry at Apple, but they have had this pretty major bug for a while now, and apparently, it affects a lot of us Voiceover users.
Quite annoying.

Once I had this problem and the solution for me appeared that simple. Siri and system/VO volumes operate at different levels. So possible that Siri volume is at the audible level and system/VO volume has got down to zero.
