VoiceOver crashed and reset settings to defaults when making a phone call

I've come across an annoying bug which I'd like to know if other people have come across. My phone crashed today when I was making a call. I tried ending the call but it wouldn't let me. VoiceOver got deactivated and I couldn't start it up using the triple click of the home button. It reset the VoiceOver settings to the default ones when I finally got it talking again with sighted assistance. I'm using IOS6 and Iphone 4s. This is the first time this has happened but it is v annoying especially as I was travelling outside when this happened. the synthesiser also sounds different now for some reason like it has a sore throat or something. I'm not sure why that is as I'm using the Austrailian synthesiser.

My friend had this happen to her twice. Once it was at a very inopportune time while she was at the airport. I believe she was also making a call. Seems to be difficult to pin down, as it occurs randomly. She was able to re enable VoiceOver via iTunes.
I tried via Itunes to re-enable VoiceOver but it didn't work. It crashed when I tried to end the call I think when I doubled tapped on the screen.

My crash was not caused by making a call, but I got the same outcome: voice over was gone, and when I got it back it was the compact voice. You will need to go into the voice over setting and toggle the compact voice on or off setting.
