My go to app for scanning bar codes is Digit-Eyes. Not only will it give you the item name, but also product information like ingredients and cooking instructions. It can also be used for other items that come with a bar code, and items you identify with a bar code. It's an essential app on my phone.

For text, I use KNFB Reader. I haven't tried to scan a picture of a document as you suggest, but I think with the new feature that allows it to scan from the camera roll, I think this might be possible.

Hi. I to have digit-eyes and it's wonderful. Granted the finding of a barcode can be difficult, they have tones to let you know where it is. I am hoping someone puts up a quick podcast of the tones, because though I know they are there, it is sometimes hard to know what they mean. If you want, try red laser, it's also a free app where the digit-eyes one is I think ten bucks. But you get wonderful support from the comapny who makes digit-eyes. As for knfb reader, that's a 100 dollar app.
