I have an iPad Mini 5 running iOS 18.2.1, and a HumanWare NLS eReader, pretty much the same device as the APH Chameleon and the Brailliant BI X 20. For a while now, it has been randomly disconnecting and then reconnecting again after a few seconds. When this happens, the braille display says its connected throughout, but when it reconnects VoiceOver makes the display connection sound. However, it does not make the disconnection sound when it disconnects. Even worse, sometimes it does not reconnect for up to 5 or 10 minutes, and activating "Reconnect devices" on the Braille display does not do anything, and neither does shutting down the display and turning it back on. Restarting VoiceOver does not help either. When I open VoiceOver Braille settings and activate "More info" on the device, the Settings app stops responding until I close it from the app switcher. Turning Bluetooth off and back on doesn't do anything either. When this happens, the only solution I have found besides waiting is restarting the Ipad, and then it always starts working again. I have tried factory resetting the Braille display, and forgetting the device from the iPad, but the issue still persists.

It's not the same device but I am experiencing really flaky behaviour with my Vario Ultra. It seems, when I wake it from deep slumber, it won't connect to iPhone, again, if I try the brail display reconnect, toggle bluetooth, repair device, it doesn't work until I reset my phone.
I'm sorry you're having this issue but I'm also slightly relieved it's not just me. I thought it was just because my display is old, but it sounds like yours is one of the latest gen?

Maybe, an update is available for your eReader? I have one. I've never used it as a Braille display because I also have a Brailliant BI 20. Make sure your iPad is also running the latest version.

I updated my iPad to the latest version (18.3) yesterday, and it still happens. There have been no updates for the eReader for a while, even though the Brailliant, Chameleon, and Mantis have gotten updates. NLS does not distribute all the HumanWare updates to the eReader, even when they include bug fixes, and when they do the eReader updates come later, and even then they don't include all the features, like the eReader still doesn't have TTS or audio playback like the Chameleon and Brailliant do. But maybe I can manually update the device from a file, since the firmware is the same between models, and it seems to just use a value encoded in the ROM to tell which device its running on.

I'd be careful. If it has slightly different hardware, which it sounds like it does if it doesn't have audio, and it might be a less powerful processor, you could end up bricking it.
I agree it is very frustrating, but it might be something you can seek professional assistance with.
