I've not found it to be 100% reliable, but it should be VO modifier with H. Hold donw the VO modifier key and press H twice quickly. Sometimes, it will go to the Home Screen, but usually this brings up the App Switcher. Your keyboard may have a dedicated key for this or it may not, but that is the command used with VoiceOver. I have found that this command sometimes conflicts with "full keyboard access" which is anything but full access when VoiceOver is running.

In my keyboard, command arrow up opens app switcher.
