I rarely do this with my phone but is it not possible to send attachments directly from the native mail app using voiceover? I had to go to the sharesheet of the file that I wanted to send and send it that way. It seemed to me at one point that I could do this from the mail app itself. I scrolled all over the place and could not find an option but I know sighted users are able to do this. Running IOS15.6 on an IPhone SE third gen.

Expand Toolbar
This is what you would be looking for. The toolbar becomes accessible only when the message body is editting. Either swipe right once from the message editing field, or explore above the letter P of the active keyboard. Hth.

On that toolbar, there's 2 or 3 butons. Insert drawingeascan document, and insert attachment. Which ever type of attachment you're sending, will determine which button you click on. I suggest finding the letter P, and swiping until you find that toolbar. It'll be much easier to find it that way.
The way to send attachments isn't obvious. The first time I had to do it, I had to call Apple's accessibility support. I'm like you, I don't send attachments often. But, when I have to send them, I do. Whether it be a screen recording, screenshot, or document.

I'm using the iPhone XS Max running the same iOS version. What I do is simply find the letter Q and swipe left from there. You will see many options, such as insert drawing, insert attachment and so on.
Copy and paste
Like with most things Apple makes things so simple that it can be hard. Try just copying the file then pasting in the message body.
