I have reported this to Apple, but noticed in the main post about iOS 16, that others only see the application switcher bug when invert colors is enabled. On my iPhone 13 mini when voiceover is on, I cannot open the app switcher even though I have turned off both invert colors and dark mode. Is anyone else still seeing the app switcher bug even with invert colors and dark mode both turned off and voiceover on?

No, but other appswitcher bugs still persist.
I open appswitcher, scroll to top. My apps at the top are weather and Notes. focus lands on the dock for a second, then jumps to the top at Weather. I swipe left, and it clicks but without speech. Then it says Notes, then Weather again. I swipe back, to Notes, then Weather is twice before landing at the top again. then I open notes, it says "Notes Bard Mobile" (Bard Mobile the app I was on before I opened app switcher) like it was opening split view even though it wasn't,, I checked. I closed it again and it still says notes at the top for a second, then same thing: Weather, click-with-no-speech moment, itunes, Weather, Swipe back, Itunes, Weather, weather. I can't close the first weather icon but I cclose the second one. I think the bug with appswitcher duplication still exists just with the first item.
As for your bug, try restarting your phone. I don't use Dark or Invert colors but I remember a bug in IOS13 for some people who used Invert, where the screen would go white for a second when waking up the phone. Turning off Invert didn't solve it, but if you turned it off and restarted the phone it would. Maybe it's just like that. You need to maybe Turn off VO and Invert, restart the phone, turn VO on, and see what happens.

I experience exactly the same bug, and as you say, it seems to have no bearing as to whether smart invert is on or off. as an aside, smart invert keeps turning on automatically, and I have worked out. It only happens when the phone is restarted.

That's really strange. It doesn't happen to me. I'd say all who are experienceing this bug report it to Apple, because if they get more than one complaint they're less likely to disregard it.
