It is not a problem with what app you use, it is about VoiceOver being very bad with text editing on iOS and iPadOS. I tried iAWriter, VoiceDream Writer, Notepad for iPad, Pages, Google Docx, Pages, Microsoft Word, Scrivener, the native Notes app, and many other apps. All have very similar problems.
This is one of those things that's better with talkback and external keyboards. That said I don’t think you’ll have a good experience really anywhere. If you’re a braille user one of those smart displays with little basic notepads might do the job but you won’t find a good solution with voiceover on iOS.

Best sticking to Mac if you can
Based upon the recent discussion in the following thread, you aren't likely to find a text editor that works well with an external keyboard:

Hopefully somebody can suggest an app that will get you somewhere close to what you want. Just don't expect anything close to a Mac experience.

I tested stuff on schools mac mini.
better in the android world but a computer is still best
