Has anyone else encountered this Kindle app problem?

I started listening to a series I found that had been out of print for years. It is a long series, about 35 books and somewhere areound book six or seven I ran into a problem that I thought was title specific, but that now seems to be a consistent problem with all Kindle books. What happens is that there is a long pause at the end of each chapter and then the menu that is supposed to pop up at the end of each book requesting that you review the title now that you are finished pops up. It pops up at the end of each and every chapter, and one has to close it and then close the menu that usually pops up if you pause medbook in order to get back to the content. Audible suggested that I return the book that was doing this and that I then rebuy it. Luckilly, I put off doing this and discovered that the next book did this as well and that any other Kindle book I listen to behaves the same way, so returning and rebuying is not a solution. Unfortunately, this leaves me looking for a solution that will work. I am using an iPhone 4 with the last release of iOS 6 on it. I plan on updating to 7 soon, but would like to see if I can fix the problem with my current setup first if at all possible. A second problem has also plagued me since I first got the Kindle app, back during the same week that the first accessible version was released. Since I first registered, my iPhone has shown up as Chris's 2nd iPhone. I've never had any other iPhone either registered or unregistered with the Kindle store. About a month ago, an entry for Chris's 3rd iPhone has appeared. Again, I am still using the same single iPhone. Except for my own OCD need to have devices properly named, there was no real issue with the listing for the 2nd iPhone, but I am planning on updating to an iPhone 5s in the next month or so and I want to remove the old phone and add an iPad and an Android phone to the account as well. I do not want to run out of slots for devices just because the Kindle store is stupidly seeing all of my devices as two devices. So, I need to learn the easiest way to remove the second iPhone that does not exist and that is listed as the third and to have the first iPhone which is listed as the second relisted as the first until I have the new phone and then I need to know how to permanently remove that one from the account. This should be so easy, but since nothing has been listed correctly from the very beginning, I don't have a lot of confidence in anything goingly smoothly with this.
