App switcher not receiving voiceover focus on iPhone SE 3rd Gen

iOS version: 26.3.1

I upgraded from iOS 18 to iOS 26 a couple weeks ago and things have been working fine, but over the past few days, seemingly out of nowhere, I’m experiencing a strange issue. I will double tap my home button to bring up the app switcher and voiceover will announce “app switcher,” but the app switcher doesn’t appear. I’m still on the home screen with all my apps. If I turn off VoiceOver and turn it on again and then pull up app switcher it works, at least for that time, but then I have to repeat this step of restarting voiceover every subsequent time. Has anyone else experienced this? I will also contact Apple Accessibility to make them aware, but wanted to see if anyone had any workarounds or remedies in the meantime. Thank you

When you double press the home button to access the app switcher, are you just waiting for VO to focus on it?
Have you tried touching a finger to the lower portion of your screen (like just above the home button) to see if you can focus on any items within your app switcher?
Have you tried installing the latest update, iOS 26.3.1(a)?

Yes, because typically when I activate app switcher, then touch anywhere on the screen, I would hear the first app being displayed by the switcher. I’m guessing maybe the position/layout of switcher might be different in iOS 26 and maybe that’s what’s throwing me off? What seems to happen is, voiceover will say “app switcher“ but then when I touch the screen, I’m just hearing the names of the apps on the first page of my home screen, as if nothing is there. Between this and your first question, I’m thinking maybe app switcher doesn’t take up the whole screen anymore and I need to poker around a bit more to find it. As of this morning, I went into software update, pulled down to refresh, and it says I’m up-to-date as of the version I included in my initial post. I will go back and try again. I’m not enrolled in beta updates though, so I’m only going to see major releases.

Have you restarted your phone at all after the update? Sometimes it fixes some weird issues. Although the suggestion from Brian to put your hand on the center of the screen is a good one too. I’ve had to do that before the same model phone hope this helps.

I had the same issue as I was clearing my apps. This is a guy with a pre-owned phone.
