You can use the custom element labeller with VoiceOver to add a description or whatever you want. To do this, select the photo you want to label, double tap with 2 fingers and hold them their on the second tap for about 2 seconds. You should then hear 3 short tones, then VoiceOver will say "label element" and bring up a text field. Type whatever you want the photo to be described as and click done. VoiceOver should now read the photo as whatever you typed in. Hope this helps.
Thanks for the info, but I was wanting my description/location to show up for everyone, blind or sighted, say, when I share it on social media. Is there a way to do this?
I don't know exactly how you would do this in that case.
If you are sharing on social media, I only use Facebook and Twitter so I don't know about Google+ or whatever you may use, but when you click share on Facebook for example, there is a box to type a description of the photo which will show up to everyone along with the photo on their facebook newsfeed. There is also an option to tag your nearby locations, so if you took a photo somewhere you could get it to show that place aswell. If you don't use Facebook this might sound gibberish so sorry about that it was just what I thought of :)

Wouldn't you turn on your location services. So that the GPS can track your position and have it show up in your photo. I am just guessing here since I turned all that stuff off because I personally don't like the idea of my position being how you might say broadcasted to the world but yeah turn that stuff on if its on already then I am sorry I am unable to be furthur help. I wish you all the best in achieving your goal though.

I really like the custom labeling feature on VoiceOver, which is extra useful for photos, but I've noticed that sometimes, it doesn't work. When I try to label a photo on the camera roll or the photo stream, VoiceOver loses the label after a while. Is anyone having this issue? Last time I tried it, I was running iOS 6.1.0, not sure if it was fixed in a more recent release. Has anyone tried it in the latest iOS update? Whether its iOS 6.1.4 on the iPhone 5, or iOS 6.1.3 on older devices?

If you get hit by the VO reset bug, which can happen when you do a soft reset of your device, you lose all VO settings including custom labels. That's the only time I've noticed the loss of a custom label.
