Hi all, posted these to Apple but wanted to make you all aware as I don’t think I saw this major issue here.
On iPhone 12 Pro I just updated last night to iOS 16.0. All seemed fine until I completely powered off the phone. Later on boot I could not tell if the phone had booted. Eventually I determined it did however VoiceOver didn’t start. Triple press of side button didn’t work nor did Siri since the first passcode login after boot did not occur yet. This required sighted help to enter the six digit passcode; after which Voice immediately started up.
Next more minor bug is the App Switcher isn’t working. From either home or in any app the swipe up to the 2nd sound point doesn’t perform any action. Triple press of side button, the swipe up motion then triple tap side button to re-engage VoiceOver does put you in the Appp Switcher.
Last bug is in Edit Mode I.e. double tap and hold icon on Home Screen. Then go to the page 1 of X above the dock to double tap and list all pages doesn’t allow a drag of a page to a new location. Moving any page to any other location and dropping doesn’t actually move the page when Done button is pressed.
I’d say the first issue is a blocker to anyone thinking of an update. Had I needed to call an Uber to get home I would have needed to find a stranger or kept trying my luck at a totally blind precision tap of six digits to unlock for the first time.

ran into this bug for the first time yestorday
I never came accross this bug when I was beta testing... until yesterday. Oddly enough, it happened when I was connected to my airpods pro, not sure if this has something to do with this? But I rebooted my phone again just for testing purposes and it didn't happen, this was with my airpods disconnected at the time of reboot.
Edit, just rebooted again with airpods on while rebooting, and the phone booted up as normal with voiceover turning on automaticly after startup. This bug seems to happen infrequently and it seems to be very hard to trigger intentionally making it hard to fix imo.
I am still beta testing, and I have not run into this issue yet.
I am currently beta testing iOS 16.1 and I have not come across this issue yet. Everything seems to be working normally for me.
I don't use invert colors, so I am not having problems with the app switcher either.

I agree this should be marked as a serious bug and don't understand why it's not marked as a bug at all in the bug report. Apple accessibility sent me a diagnostic link so that if I reproduce this issue I could upload the log. To be honest this is one bug I hope I don't reproduce. I mean yes apple I understand does need the report, however I don't have sighted assistants.
As odd as this may seem when I set up my new IPhone SE3 earlier this year under ios15, I experienced this very same bug when powering off the phone and powering it back on so this makes me wonder if this bug has been around for a while and is just one of those bugs that's so rare therefore making it hard to reproduce.
Not encountered that ONe, But Have Encuntered another One.
I've not run across that one, but I'm experiencing another one altogether that crashes Voiceover repeatedly any time it thinks the phone is doing too much, such as if you have two mapping apps open at the same time, or if you have the nerve to open the breaking News app. And once it crashes a certain number of times, it defaults the voice to one I didn't choose as my default voice, which forces me to have to go back in and reset it to the voice I want, only for it to do it again a few minutes later, if I'm lucky, a few seconds later if I'm not.
