g’day all.
One of the problems that I have noticed given that I wear hearing aids, is that connecting any other device when one devices connected is extremely problematic.
I suggest that if you have a Bluetooth device connected when you’re attempting to use the speaker you disconnect, forget the device on the iPhone, and repair. Do the same for the speaker if necessary.
I hope it works for you, I’ve had all sorts of problems trying to connect my braille display to the iPhone even after successful pairing.

I had Problems using Siri with a different speaker and no other devices were paired at that time. I could barely hear Siri voice it's prompts and responces. The Siri volume started very low. But, that wasn't the only problem I had with Bluetooth and iOS 13. Some devices work properly but many don't.
I spend hours with Apple troubleshooting, screen sharing, sending logs, and duplicating my experience. Voiceover would not go through Bose Beets and other speakers. Sometimes the Voiceover voice and other sounds would be very fast and high pitched at first.
I'm not sure how or why Apple has so much trouble with Bluetooth but it really bugs me. speakers would not
