Your Feedback is Important to Improving Accessibility on Apple platforms
Don't assume that Apple is aware of a bug or that your report won't make a difference - submitting bug reports directly to the company helps them reproduce and resolve the issue faster. Your report may provide crucial information. The more reports they receive, the higher the priority they give to fixing the bug.
If you're using a beta version, use the Feedback Assistant on your device to submit feedback and bug reports. If you're using a public release, follow the instructions on this page.

You probably have to reset settings
It’s always a good idea to reset your settings once in a while. You also would benefit from contacting Apple accessibility to report this issue.
Mysteries
I don't know if it's due to me never using iOS betas or something but I never had these focus problems. Sure, VO sometimes jumps around the screen, but it doesn't get to the point where I'm actually annoyed. Every program gets a bit bonkers once in a while.
Contact Apple accessibility by phone.
As Ash said, and has previously said on other threads. Apple don’t seem to be aware of just how bad voiceover has got. I have been having a completely miserable experience with it for the last year. Mine has got quite a lot better since iOS 17 beta, me clearing my storage down to under 50% re-installing a ad blocker. Focus still jumps regularly and I’m still not happy with how things are but those changes did help to the point that I don’t literally hate my phone and having to use it anymore which is a nice improvement. I also tried resetting all settings but it didn’t do much for me. Still worth giving it a go though. It might take an hour to get everything back how it was but you never know, it might fix problems so an hour is definitely worth it. Failing that you could consider a completely fresh install so resetting to factory and not restoring from backup. Literally starting like you’d never had an iPhone before. Obviously thats much much more work and you’d probably lose messages but it’s always an option. I’d go for the Apple phone number if I were you, especially if Ash is right and Apple just aren’t aware. I’ve made them aware too but the more the merrier. It definitely can’t hurt. They are providing a contact team for a reason so lets make them notice us and take these problems seriously.
Factory reset
I'm seriously considering a factory reset when upgrading to iOS 17.
Back in the day I always performed a factory reset after upgrading to a major release.
I like to believe that this practice saved me from a lot of issues and in fact I rarely experienced problems.
PS: you don't loose text messages / iMessages as long as you store messages in iCloud.
Not iCloud backup but by toggle the option to allow Messages app to store data in iCloud.
Messages app will sync when reset done; you may need to toggle iCloud storage again though to let Messages app to sync your messages from iCloud.
NO
13 pro. No issues with focus not even when I was using 12 pro. Hope some of the suggestion work for you.
